Simplified Pulmonary Embolism Severity Index (sPESI)

Age >80 years old (1 point)

History of cancer (1 point)

Chronic cardiopulmonary disease (1 point)

Pulse ≥110 beats/minute (1 point)

Systolic blood pressure <100 mmHg (1 point)

Oxygen saturation <90% at diagnosis of P.E. (1 point)

Total Criteria Point Count:

sPESI Score Interpretation

0 points: Low risk of 30-day mortality
1 to 6 points: High risk of 30-day mortality

Notes
  • In the Lankeit study of the Simplified PESI an sPESI score of 1 or more was predictive of high risk of adverse outcomes with a sensitivity of 94% and specificity of 40%
  • The high-sensitivity troponin T test was also found to predict adverse events when hsTnT is ≥14 pg/mL with a sensitivity of 87% and specificity of 42%
  • When both sPESI and hsTnT high risk indicators were present, the sensitivity rose to 100% but specificity fell to 26%
